Free Students Union (FSU) is the only students' society recognized by government of Nepal .It is present in all colleges under Tribhuwan University run by government, which is oldest among universities in Nepal . Regarding Free Students Union, Maharajgunj Medical Campus, is the only students society recognized by government of Nepal all over the medical institutes in Nepal .

It was established since the establishment of this institute . It is formed by elected students among different panels being voted by students of IOM ranging from medical students, resident doctors, allied medical science students, of different faculties from certificate level to postgraduate levels, not only the Nepalese students but also the foreign students studying in IOM, Maharajgunj Medical Campus. So, it works as a mother organization for all other students societies like Nepal Medical Students Society(NMSS), Nepal Pharmacy Students Society(NPSS), Nepal Public Health Students Society(NPHSS), Nepal Radiology Students Society(NRTSS), Nepal Medical Laboratory Students Society(NMLSS), Nepal Ophthalmic Students Society(NOSS),Nepal Resident Doctors Association(NRDA) etc.

It conducts different curricular as well as extracurricular activities- Welcome Programme for newcomers, Graduation Felicitation Programme for graduate students, Annual Sports Festival , and supports to other students societies in different programmes. It acts with the sole aim for welfare of students of Institute of Medicine , Maharajgunj Campus. The executive body of FSU is elected every two years by an election conducted usually in the month of Falgun.
